As Tennessee leads the United States in coronavirus cases per capita following a post-Thanksgiving spike, the state’s health commissioner warned Sunday that the hospital system will crumble if residents don’t slow the spread of the virus by staying home, wearing masks and avoiding large gatherings.

“If we have another surge over Christmas, it will break our hospitals,” Tennessee Health Commissioner Lisa Piercey said at a news conference, according to the Tennessean.

Despite Piercey’s warning, Tennessee Gov. Bill Lee, R, refused to order a statewide mask mandate in a video address hours later. Instead, he announced a new executive order to limit indoor gatherings to 10 people and implored people to keep holiday gatherings to just those in their household.

“I understand deeply how much Tennessee families need each other,” Lee said. “But we must do all that we can to blunt this surge and keep more Tennesseans from getting sick.”

The executive order comes one day after the governor’s office announced that Tennessee first lady Maria Lee tested positive for the coronavirus after exhibiting mild symptoms. In a statement, Lee said he tested negative and would quarantine as a precaution.

Tennessee is averaging almost 10,000 new coronavirus cases, and almost 100 deaths, per day, according to The Washington Post’s tracker. About 2,800 people in Tennessee are hospitalized for covid-19, the illness caused by the novel coronavirus, with about 760 in the intensive care unit, according to the Tennessee Health Department. On Saturday, Tennessee recorded a 30% positivity rate, a new high, according to the Tennessean.

In the weeks following Thanksgiving, daily reported cases in Tennessee doubled. A similar trajectory played out across the country, and health officials warn that Christmas gatherings could lead to an even larger surge.

Despite the rapidly increasing numbers pushing Tennessee hospitals to their limits, Lee maintained there would not be a mask mandate, while stressing the importance of wearing one.

“Many think a statewide mandate would improve mask-wearing, many think it would have the opposite effect,” Lee said Sunday. “This has been a heavily politicized issue. Please do not get caught up in that and don’t misunderstand my belief in local government on this issue. Masks work and I want every Tennessean to wear one.”

Lee added that 70% of Tennesseans are already under mask requirements from their local governments and 80% said they wear their masks always or most of the time.

“And I thank them for doing this,” Lee said.

But medical workers criticized the governor for not going far enough to help them during a time in which they are drowning under the increasing hospital rates. Following Lee’s address, the Tennessee Medical Association, a nonprofit advocacy group for state physicians, released a statement imploring the governor to change his mind.

“We believe all Tennessee counties should be under mask requirement orders at this time,” the group said in statement, WVLT reported. “We plead with all Tennesseans to stay safe, stay apart, wear masks, and stay home to protect their families and friends from this deadly virus.”

The pressure on Tennessee health-care workers has been so great that Lee said he had authorized National Guard medics to work in hospitals to “provide lifesaving care.”

During Piercey’s news conference Sunday, the health commissioner said five of the state’s largest health-care systems are allowing paramedics to practice inside the hospitals. She added that with the exception of hospitals, health-care workers who tested positive for the coronavirus could continue to work in long-term-care facilities if they felt well enough.

“I tell you this because we are looking under every rock. We are turning over every stone to help hospitals,” Piercey said. “We are running out of options.”

The survival of the state’s health-care system relies on Tennesseans staying home, wearing masks and avoiding gatherings, Piercey said. But she worries that people will fall back on false assumptions that having close friends or extended family at their homes is better than going to a bar.

“That’s a dangerous mentality,” Piercey said.

At the end of his address, Lee acknowledged the state is facing Its “darkest hour” but offered hope that if every Tennessean complies with the state’s health directives, they will get through the next few months.

“We are in a cold, cruel phase of this pandemic,” Lee said. “It will get worse before it gets better. I know you are tired. But we have got to double down.”

Republicans brimmed with pride over the $900 billion stimulus package taken up by Congress on Monday, with Senate Majority Leader Mitch McConnell calling it “a historic bipartisan rescue package” and Sen. Dan Sullivan of Alaska predicting it would be remembered when “the history of this challenging year is written.”

President-elect Joe Biden saw it differently, calling the deal merely a “down payment.”

Biden’s assessment, made repeatedly in recent weeks, echoes the view of many Democrats who see the bill as the beginning of a negotiation, not the end. The notion that more aid is necessary – Biden at times has said talks should start as early as January – sets up perhaps the first major legislative test for the new president and his self-proclaimed negotiating skills.

It’s a challenge made greater now that Congress has approved new spending in the areas where common ground was relatively easy to find. There is also an expectation that many Republicans will become more skeptical of big-ticket spending after President Donald Trump leaves the White House.

Biden has consistently said that bolstering the economy will be one of his four top priorities – along with ending the pandemic, addressing climate change and promoting racial justice – suggesting he will put a strong emphasis on new spending. He has been vague about what he would like in a new package, but it would probably include aid to cities and funds to prepare for opening public schools.

That push, however, will clash with an energized Republican Party eager to reestablish its fiscally conservative bona fides. McConnell, R-Ky., has signaled that he recognizes additional negotiations are likely but said it would be difficult to finalize another package.

Privately, many in Biden’s party agree it will be hard to persuade Republicans to reopen the debate after the protracted, often tortuous negotiation that led to this last-minute deal days before Christmas.

Biden declined to take detailed questions from reporters Monday after he received his coronavirus vaccination at Christiana Hospital in Newark, Del. But an official on Biden’s transition, who spoke on the condition of anonymity because the person was not authorized to speak publicly, said the president-elect wants additional money for “supporting the covid response effort, reopening schools and helping families, businesses, and state and local governments.”

Biden has been blunt in recent weeks that he considers Congress’s current package far from enough to tackle the widespread economic damage caused by the pandemic. “I’ve made it real clear,” Biden told reporters in Wilmington, Del., earlier this month. “This is not the end of the deal.”

Senior Republicans viewed Biden’s victory as key to unlocking the current deal, because it gave Democrats a credible justification for backing off their previous multitrillion-dollar demands that GOP lawmakers had no intention of meeting. Chief among them was their insistence that struggling state and local governments badly need a cash infusion to help provide vaccine distribution and other services.

But for many Republicans, the approximately $900 billion package that came together this week represents the outer limit of what they are willing to accept.

Furthermore, there is no obvious deadline to compel action, often a requirement on Capitol Hill. The bill was forced to the finish line by a looming government-shutdown deadline and a pair of Senate runoff elections in Georgia on Jan. 5, as Republican leaders were anxious that the lack of a relief package was hurting Sens. Kelly Loeffler, R-Ga., and David Perdue, R-Ga.

The clamor of rank-and-file lawmakers faced with returning home for Christmas without a deal in hand also created pressure.

Similar pressures are on the immediate horizon next year. Instead, the impetus for further relief, if any, is likely to be driven by the pace of the economic recovery and whether employment and growth rebound in the weeks after Biden takes office.

The president-elect has suggested the economy may not rebound immediately, saying that would drive Republicans to the negotiating table. “As things get worse, they’re going to find that there’s an overwhelming need,” Biden said recently.

One potential deadline comes in mid-March, when 11 weeks of additional jobless benefits provided in the current bill to the long-term unemployed are set to expire. But several Republican senators have signaled that even then, further action is doubtful.

“If we address the critical needs right now and things improve next year as the vaccine gets out there and the economy starts to pick up again, you know, then there’s maybe less of a need,” Sen. John Thune, R-S.D., said last week.

One demand, however, could bring Republicans back to the bargaining table: their desire for a sweeping “liability shield” barring coronavirus-related lawsuits against businesses. GOP leaders, in the most recent round of negotiations, sought to pair that with a top Democratic priority – tens of billions of dollars in direct aid to state and local governments. In the end, both issues were dropped from the talks.

Although some Republicans have broken with party leaders to call for additional state and local aid, Democrats remain dead set against immunizing businesses from lawsuits.

Months of talks on the subject sputtered, and it could be difficult for Biden to make headway on it. But the liability issue may be his only chance to force new action, even if Republicans are unlikely to trade it for anywhere near the trillion dollars of additional stimulus Democrats want.

“I think everybody understands that Vice President Biden is going to ask for another bill, so we will have another chance to revisit it probably pretty soon,” Sen. John Cornyn, R-Texas, a key proponent of the liability shield, said Monday.

Biden has largely avoided naming specific items or a spending target for the next package, saying that would be a bad negotiating strategy. “I’ve learned, after hanging around in this business for a while, the last thing you do before you begin a negotiation is lay out a drop-dead marker,” Biden said earlier this month.

Asked if he believes the right number for the next package is in the billions or trillions, Biden responded: “Hundreds of billions of dollars.”

Still, Biden has sketched out some of his priorities over the past few weeks, including additional funding for state and local governments. The current package includes none of this spending despite pleas from states such as New York and Illinois, whose governors have warned that they may need to raise taxes, lay off public-sector workers or cut spending on critical government programs to break even entering next year.

Biden has cast the state and local aid as critical for jobs, saying it is important that the federal government provide help to local governments to prevent layoffs of public employees including teachers, firefighters and other first responders.

Biden also said recently that “it would be better” if the deal included $1,200 checks for low-income people. The current bill includes $600 checks for people making less than $75,000 a year – which alone cost $164 billion or about as much as the new state and local aid Democrats have sought.

Democratic leaders agree. In a statement late Monday, House Speaker Nancy Pelosi, D-Calif., penned a letter to her colleagues about the legislation that laid a marker for the next round, noting that “state and local budgets are still overwhelmed by coronavirus expenses and revenue losses.”

Biden has also said Americans should not be evicted from their homes during the coronavirus crisis. The current compromise includes a measure that extends a moratorium on evictions through the end of January.

Biden has said extending unemployment insurance is necessary “so they can continue to feed their families.”

This week’s package allows for a $300-per-week additional unemployment benefit until at least mid-March, about half of the $600-per-week additional benefit that Congress enacted last spring.

The bill also extends a special program for gig workers.

Asked whether he would have any leverage to bring Republicans skeptical about the price tag back to the table, Biden said Americans will be desperate for more help.

“The country’s going to be in dire, dire, dire straits,” Biden said. “We’ll be in dire trouble if we don’t get cooperation, and I believe we will.”

Biden also offered a smidgen of bipartisan bonhomie on Monday, perhaps practicing his negotiating skills. As he received his vaccination, Biden praised the White House, saying, “I think the administration deserves some credit for getting this off the ground.”

Pattaya has cancelled its New Year countdown event this year in a bid to curb the possible spread of Covid-19, said the city’s mayor Sontaya Kunplome on Tuesday.

The cancellation follows an outbreak in Samut Sakhon that has so far spawned more than 1,000 cases and spread to nearby provinces.

Chonburi governor Pakarathorn Thienchai, who chairs the provincial disease prevention committee, has held a meeting with related agencies on virus-control measures. As a result, he issued a ban on all New Year events in the province, including the Pattaya countdown.

The Pattaya Countdown 2021 was expected to draw about 300,000 visitors to the seaside city from December 29-31.

However, visitors would still choose Pattaya for their New Year holiday despite the cancellation of festivities, Sontaya added.

The authorities have released an official timeline of a 24-year-old Chachoengsao resident’s movements after she tested positive for Covid-19 on Tuesday.

The woman lives and works in Chachoengsao’s Bang Pakong district and went to work every day between December 7 and 11.

On December 12, the woman, her boyfriend and friends had a meal at a buffet restaurant in Bang Pakong’s Bang Wua area.

On December 13, she travelled to her second house in Samut Prakan district and visited the Mega Bangna complex. She also ate at a restaurant near Assumption University’s Suvarnabhumi Campus before returning to Chachoengsao.

On December 14 and 15, the patient went to work at her office and dined at a restaurant in Bang Wua sub-district.
On December 16, she only went to work.

On December 17, the woman ate at an eatery in Bang Wua area and later visited a pharmacy and local market near the Bang Wua branch of Tesco Lotus.
On December 18, she went to office.

On December 19, she visited a Toyota service centre in Samut Prakan’s Bang Bo district, before visiting her boyfriend’s home in the same province. The patient then dined at a buffet restaurant called “Nari” in Chonburi before returning to her home in Chachoengsao.

On December 20, the patient returned to Samut Prakan to cast her vote for the provincial administration organisation’s chief at the election station in Bang Phli Noi market. She also visited Chularat 11 Hospital, her boyfriend’s home and a 7-Eleven outlet in a PTT petrol station.

Thirty-day visas have been extended by 15 days to cover the time needed for foreign visitors to stay in quarantine. The Cabinet on Tuesday approved the visa extension to 45 days for short-stay visitors, said government spokeswoman Traisuree Taisaranakul.

The move was proposed by the Interior Ministry since foreigners who enter the Kingdom on a 30-day visa or visa-exempt stamp must currently quarantine for 14 days.

Thailand’s domestic tourism market has been rocked by the outbreak of Covid-19 in Samut Sakhon province that has so far spawned more than 1,000 cases and threatened New Year celebrations across the country.

Marisa Sukosol Nunbhakdi, president of the Thai Hotels Association, said the surge in infections has hit hotel bookings for New Year holidays. Many tourists have cancelled advance bookings, leaving hotels to foot the bill for singers, performers and DJs already booked for New Year celebrations.

The outbreak is expected to hurt the hospitality business in Bangkok and Hat Yai in the South, both of which are heavily dependent on foreign tourists. In Bangkok some hotels are bustling, but many have remained closed since the start of the virus crisis in early 2020 and are unlikely to open soon given New Year countdowns may be cancelled.

New Year domestic tourism in secondary cities such as Nakhon Si Thammarat, Nan, Sukhothai, Ayutthaya and Pathum Thani will likely be less badly hit. But hotels in Buriram, which is reliant on organised events, are expected to suffer a heavy impact over New Year from the outbreak.

Yuthasak Supasorn, governor of the Tourism Authority of Thailand (TAT), said that before the Samut Sakhon outbreak, domestic tourism was on course to hit 100 million trips in 2020. That target has now been lowered to 95 million trips, generating income of Bt500 billion. Meanwhile the number of foreign tourists this year is expected to be around 6.7 million people, generating revenue of Bt330 billion, for total annual tourism revenue of Bt830 billion.

Yuthasak said tourists still wanted to holiday in popular destinations like Phetchaburi, Hua Hin and Prachuap Khiri Khan at New Year, but might have to adjust their travel plans to avoid risky areas like Samut Sakhon.

“Many may also change their travel plans to long-distance destinations that require a flight for the New Year’s holiday,” he added.

TAT’s latest tracker report shows that just 1,201 foreign tourists entered Thailand in October, a fall of 99.96 per cent compared to the same month last year. Meanwhile domestic tourists in November totalled just over 16 million (16,017,165), an increase of 10.4 per cent from a year earlier and up 2.63 per cent from October.

November’s hotel occupancy rate was 37.56 per cent, a fall of 32.77 per cent from a year earlier but up 3.5 per cent from October.

The Chiang Mai Office of Disease Prevention and Control 1 as well as Chiang Mai Municipality officials began collecting samples for Covid-19 tests from migrant workers in the Muang Mai market area on Tuesday.

Muang Mai is the largest seafood and fresh food market in the province.

Some 2,500 people work in the market, of whom 1,500 are migrant workers. The market serves no less than 10,000 customers daily. However, since the Covid-19 outbreak earlier this year, there have been no new workers or transfer of labour in the area.

Meanwhile, some 200 people were seen queuing up for tests on Tuesday.

Dr Chatuchai Maneerat from the Provincial Public Health Office said the recent outbreak in Samut Sakhon has raised awareness in Chiang Mai about high-risk groups. Vendors who get their supplies from Samut Sakhon’s Mahachai area as well as migrant workers living in cramped conditions are most at risk.

Separately, results on more than 300 samples taken at Kamthiang Market and Pa Pae community have come out negative. Though this indicates that these areas are relatively safe, full investigation is still required.

Tests will be conducted in 10 new locations, Dr Chatuchai said, adding that news of a positive case travelling from Ayutthaya province to Chiang Mai’s Mae Ai district was of great concern. Health officials have inspected at-risk areas in Mae Ai district, including the Malika and Tha Ton subdistricts. So far, 145 people have been confirmed as negative. Health officials will spend another three days collecting samples before confirming the situation.

The Public Health Ministry on Tuesday released a timeline detailing the movements of a 63-year-old man in Nonthaburi province who tested positive for Covid-19 on Monday.

The man, who lives with his wife in Nonthaburi’s Muang district, has come in close contact with 15 people, who are being tracked by officials.

According to the official timeline, the man had come in contact with Myanmar migrant workers, who worked on a refurbishment job on December 3 and 11. Some workers had a cough and did not wear face masks at all times.

On the evening of December 11, the patient attended a party held by Saksiam International.

On December 12, he attended a meeting at a private company in Rangsit area at 10am, and then dined with his wife at a restaurant in Muang Thong Thani at 7pm. By then he had developed symptoms of fatigue and was suffering from diarrhoea.

On December 13, he visited his son’s home in the Venue Flow housing estate in Muang province where he reportedly touched four members of the son’s family. His son later took him for a massage to help with his fatigue.

In the evening, the patient attended a party at Arnoma Grand Hotel in Bangkok and came in contact with 22 people. During that time, the man had run to the toilet around six times from diarrhoea.

On December 14, the patient stayed at home. He was unable to eat and passed blood in his stool twice.

On December 15, he stayed in again but held online meetings with his colleagues.

On December 16, he spent most of the day at home, but dined at a restaurant in Nonthaburi’s Prachachuen area at 7pm and at 8pm, he went to The Mall Ngamwongwan complex and entered a Nike shop.

On December 17, he travelled to his other house in Bang Yai district and returned at 4pm.

On December 18 and 19, he stayed at home.

On December 20, he took his wife to Thansamritwittaya School in the province, where she cast her vote for the provincial administration organisation (PAO) chief. After that, he went to another election station in Bang Yai district to exercise his own ballot.

At 3pm that day, he drove to Bang Bua Thong Hospital to undergo a Covid-19 check. The following day, test results showed he had coronavirus.

As of 11am on Tuesday, Samut Songkram’s public health officer Dr Santhit Booyasong said eight people had tested positive for Covid-19 in the province.

He made this announcement at a provincial outbreak prevention meeting chaired by governor Charas Boonnasa.

“The eight are among 91 high-risk persons tested after visiting Samut Sakhon, where an outbreak has erupted,” he said.

“Of the eight confirmed cases, six had visited Talay Thai market, while one visited Big C and Lotus in Mahachai, and one went to Central in Mahachai. They are all Thai nationals.

“Nine other persons are being tested for a second time to confirm the result at Somdet Phraphutthalertla Hospital,” added Santhit. “Public health officers will continue to trace and test persons who are at high risk and have prepared a temporary quarantine facility at Amphawa Hospital in case more infections are found.”

Samut Sakhon governor Veerasak Vichitsangsri said on Tuesday that the number of infected people in the province had risen to 1,063.

An engineer at the Royal Irrigation Department had himself tested after losing his sense of smell and the result came out positive. He had previously visited a market in Samut Sakhon.

The department, meanwhile, has closed its 14-storey building in Bangkok’s Samsen area for a day to give it a deep clean.

“Every employee will also be strictly screened,” the department said.

The engineer’s movements have been tracked and the timeline is as follows:

• December 13: Arrived in Samut Sakhon to dine with his family at the Krua Chaophraya Restaurant before stopping off at an ice-cream parlour in Mahachai subdistrict.

• December 14-17: Went to work as normal.

• December 18-20: Visited Si Sa Ket province to cast his vote for the provincial administration organisation poll.

• December 21: Went to work in the morning and was instructed by his supervisor to take a Covid-19 test.

• December 22: He tests positive for Covid-19.
